What we collect. Your name and contact details. The name and contact details of the person who needs care. Their Notice of Decision and Support at Home funding level, and a short description of the care they need. Whether they own their home, whether it carries a mortgage, and the address. Where someone is acting for them, who that is and what authority they say they have.
Why we collect it. To check whether your family is eligible for this care service, to introduce you to a suitable care provider, and, if you ask us to, to send Heartland Bank Australia the referral form you sign.
Who we give it to. The care provider we introduce you to, so they can assess whether they have capacity and can meet the care needs, and so they can contact you. Heartland, but only the referral form you sign, and only if you ask us to. Nobody else, unless the law requires it.
Where it is held. Vera runs on systems provided by other companies. Some of them store and process information outside Australia, including in the United States. Where that happens we take reasonable steps to see that your information is handled consistently with the Australian Privacy Principles, but it may also be subject to the laws of the country it is held in.
If you would rather not provide it. You can ask us general questions about arranging care without giving us any of this. We cannot check your eligibility or make the introductions without it.
Access, correction and complaints. You can ask for a copy of what we hold about you, ask us to correct it, or complain about how we have handled it, at privacy@vera.guide. If you are not happy with our answer you can complain to the Office of the Australian Information Commissioner at oaic.gov.au.
